Despite Underwood’s assertion that her altered appearance is a result of the accident, rumors continue to suggest that she might be using it as a cover for cosmetic procedures.
In a 2018 interview with Redbook, she candidly mentioned her desire for cosmetic enhancement, stating, "It's a little sad because the truth is just as interesting. I wish I'd gotten some awesome plastic surgery to make this [scar] look better."

Dr Dennis Schimpf analyzed Carrie Underwood’s transformation and identified several signs of potential plastic surgery. "Her nose does look like the shape has changed over time with a reduction in size making it more feminine-looking," he told Nicki Swift.
"Her skin texture looks very good, possibly as a result of laser resurfacing and or chemical peels with medical-grade skin care," he added.
Schimpf also observed that her refined cheekbones and jawline could be the result of fillers or liposuction.
Underwood has acknowledged using Botox to maintain a youthful appearance. "I do like some Botox," the 'Champion' songstress admitted on 'The Howard Stern Show' in 2023.
She also revealed her efforts to minimize the appearance of a scar above her lip. "I'm always trying to reach goals. I'm just like, 'What can we laser?'" Underwood said with a laugh.
Despite claims that her appearance changes are purely cosmetic, Schimpf notes that facial injuries can result in permanent alterations but also suspects that cosmetic procedures might be involved.
