In the clip, Underwood performed a comedic bit teasing the Affordable Care Act, commonly known as Obamacare. As they made jabs at the initiative, which aimed to make affordable health insurance accessible to more people, several stars in the audience, including Taylor Swift, Keith Urban, Faith Hill, and Tim McGraw, appeared visibly uncomfortable.

The video began with Paisley pretending to have a strained back, prompting Underwood to ask if he had tried enrolling in "Obamacare".

Underwood replied, “It’s great. I started signing up last Thursday, and I’m almost done,” drawing laughs from the crowd. The duo then walked over to a laptop, where they sang, “Obamacare by morning/ Why is this taking so long?I’m going to wind up with hemorrhoids/If I sit here ’til dawn/ We’ll have cataracts and dementia/ Oh this is getting on my last nerve.”

They concluded the song with, “Obamacare by morning / Over six people served.”

As they finished, the video cut to Blake Shelton and his then-wife Miranda Lambert laughing and clapping, before showing Taylor Swift’s stunned expression. The clip then displayed the word "uncomfortable."

At the time, Swift, who was just 23, had not yet publicly shared her political views, though she would later endorse multiple Democrats and criticize Donald Trump.
